By filling out this form you are requesting and giving permission for an evaluation of the presence of ghosts/ earthbound spirits or other energies in your house. If a presence is identified, there is no obligation for you to proceed any further. It is entirely up to you to request the removal of the energies. However we encourage you to have them removed and sent to where they actually belong.

It is important for you to fill out all of the information in order for us to accurately determine if any presence exists.
